DOI Occupational Safety and Health has created a detailed explanation for the reporting and determination of OSHA work-relatedness for all cases of COVID-19. Please review the following documentation prior to entering any accident reports or exposures in the SMIS system.

No Confirmed Diagnosis
All employees should remain alert for symptoms (including fever, cough, or shortness of breath) and maintain 6 feet of distance from others. If symptoms develop, employee should contact supervisor, self-isolate, and contact a medical provider.
No Suspected/Confirmed Diagnosis, Had Close Contact
1. Have employee self-isolate and consult a medical provider. Follow the most recent DOI COVID-19 Risk Assessment & Decision Matrix and the CDC's "What to Do If You Are Sick".
2. If the illness is work related (as defined by OSHA 29 CFR 1904.5) and the case meets one or more of the recordability requirements (as defined by OSHA 29 CFR 1904.7) - Have employee "Report Incident" in the SMIS "Employee Module".
>> Work-related COVID-19 injury cases must be reported as an incident within SMIS.
The Employee involved in the incident should initiate the accident report process by signing into SMIS using the "Employee" module. Once logged into SMIS, the employee selects "Report Incident" and completes the required fields. The Supervisor will then be notified by email of the accident and will sign into SMIS using the "Supervisor" module. The supervisor may be required to complete detailed information about the reported incident.
A DOI issued government email is required to access SMIS. If an employee does not have a GOV email address, their supervisor is required to act on their behalf to create an accident report. The supervisor will sign into the SMIS supervisor module and in the "Create New Accident Report" section, select "New Report".

Have employee self-quarantine and consult a medical provider if symptoms arise. If employee is MISSION ESSENTIAL, he/she may continue to work provided they remain asymptomatic and additional precautions are implemented following CDC guidelines.
If exposure was work related, have employee enter an Exposure in the SMIS "Employee Module".

The Authority for solicitation of the information is Section 19 of the Occupational Safety and health Act of 1970 (Public Law 91-596);5 U.S.C. 7902; 29 CFR 1960; Executive Order No. 12196 (February 26, 1980); and 28 U.S.C. 2671-80.
The principal purpose for which the information is intended to be used is to provide summary data of injury and property loss information for analytical purposes to be used in establishing programs to reduce or eliminate loss producing problem areas.
The routine uses of the information are: (1) To Provide quarterly listings of fatalities, disabling injuries, illnesses and property damage to the Department of Labor, in compliance with 29 CFR 1960.69.(2) Transfer to the U.S. Department of Justice in the event of litigation involving the records or the subject matter of the records. (3) Transfer, in the event there is indicated a violation or potential violation of a statute, regulation, whether civil, criminal or regulatory in nature, to the appropriate agency or agencies, whether federal, state, local or foreign, charged with the responsibility of investigation or prosecution of such violation or charged with enforcing or implementing the statute, rule, regulation, order or license violated or potentially violated.
The effect on the individual of not providing all or any part of the requested information may be to render impossible or to delay the Department's documenting the injury and/or property loss.
